Getting pts help they need after multiple inpatient stays

How do you help patients get longterm support. I find the ones new to system don’t get transferred to duel diagnosis when they should. We send home with meds and appts to find they end up back in within a month. Seeing more where THC is only drug they test positive for?

I’m very sorry about your son and the way they act like they’re giving up on him. That’s heartbreaking. He should be given chances as long as he wants to try.

I receive mental health services through my county. They are free and sliding scale. All my nurses do is my bloodwork and stuff like that. I have a Caseworker that does everything else.
