Committee letter Registration Questions

Currently doing the Reflection journal for my undergrad school so they can write me a committee letter. Everything was going fine until I stumbled upon these two questions:

1) How do you stay informed about the medical field

2) Describe your academic achievements to date.

The first one is weird cause IDK how to answer it. Like I want to say that I got a job working as a scribe and a dental assistant and that I used my direct relationship with the medical doctors to learn more about diseases and being a physician. But IDK if that's like answering the question

The second questions is even harder cause I don't have any achievements. Never won any awards or titles. I made the deans list for one semester when I was at Cornell but then BOMBED the subsequent semesters. My greatest achievement is getting accepted and graduated an IVY league university especially because I am a first generation college graduate that is a URM. But I don't know if its acceptable to write that. (P.S it should be worth noting that I graduated Cornell with a HORRIBLE GPA)
