What separates LORs for students accepted to Top 20s from those accepted to “average” med schools?

If anyone happens to have examples of LORs of students accepted to Top 20s, I’d appreciate taking a look at them.

Edit: I know that letter writers are encouraged to rank the applicant (top 1%, top 25% etc.). As long as the stats and EC’s of the applicant made them competitive for a Top 20 school, does it matter if their letters did not state that they were exceptionally unique/within top 1% of students theyve known?
