Not married- any issues with birth certificates?

I'm not married and it's not something that matters to us honestly. I'm in georgia and read something about if we want our baby to have his last name we'd have to go to a birth certificate office and he'd have to sign a form? Anyone have any experience or issues with this? Also Will he be on the birth certificate if we ask them to put him on there?
