How do I save my plant?!

First, I don't even know what this plant is... (sorry!) But I've had it for years and recently it's looked like this? Weird dried up glittery grey leaves that crumble into dust when touched and a smushable branch (see image 3) that's is normally rigid. What do I do? Is she too far gone?
The plant is about 6 ft from a westward facing window and is watered about every 2 weeks. Maybe I'm caring for it wrong, how do you care for this plant? Please Help!!!!
