If you take out a piercing is there always scar tissue?

I had my nipples pierced for a year and they never seemed to heal and always hurt. I really liked the look of them but I took them out anyways. Now it’s been three years and I’m thinking of getting them redone. My nipples look normal. Is there a way to tell if you have scar tissue or does it always happen with a piercing? If so does the scar tissue heal or go away after a certain time frame? I had them done horizontally last time. Would diagonally this time be a better idea or hurt less? Thanks!
