Telehealth while abroad

I've been at an urgent care that I (surprisingly) love for the past few years. Recently, I was offered the opportunity to move to our telehealth division which I'm considering. My wife isn't originally from America, and it's always been our goal to move back to her home country at some point, likely after retirement. But, I was wondering if it would be possible to practice telehealth while living in her country. Visa wise, she is still a citizen there and I could gain permanent residency under her. It would be a bit of a shift in timezones, but I'm a night owl anyway. Does anyone know the logistics or legal ramifications. I have a permanent address here for licenses and such.
