8 minute rule and Productivity Rate

I'm so confused with this 8 minute rule, I thought if we put something between 8-22 that's okay and considered 1 unit and so on and so forth. And how important is the productivity rate? Cause we have estimated time and all but what if I can't even get 55 minutes for a patient?
