Is there such a thing as HMO and Insurance in one plan?

Hi everyone! Hope someone can help. So I would like get my parents a HMO/Insurance however Iโ€™m not sure how to proceed. For context my mom is aged 45 and my dad is 47 with hypertension.

I would like to get something that can cover general check ups as well as dental health. I know usually those are HMO but I was wondering if I could get an insurance that can offer those as well?

I saw AXA GHA suggested on a diff post but I would like to get other suggestions too.

My goals for the supposed plan: ~ coverage for general check ups ~ coverage for dental health (cleaning, etc) ~ coverage for when they need to be confined due to an illness/surgery ~ life insurance in general

Is there any way I can get both benefits of HMO and Insurance in one plan or is the only way to get both? I donโ€™t have that big a budget kasi for both since Iโ€™m still trying to budget my finances due to moving out.
