Help me understand here. Its embarrassing that I work for an insurance company pero naloloka ako when I was hospitalized and knowing na may hospital allowance ako, I want to avail it. 6 days akong na confine. Though bayad naman ng HMO ko lahat and marami naman akong SL. Kinuha ko pa din iyon benefit na iyon and I am entitled to it. 1500 x 6 din is not that small.
So ayun, na weweirduhan lang ako, bakit part of the requirement is that I need to have my physician fill up a form describing my symptoms, kung kailan ako nagpaconsult or ano yung finding niya. Sure, I get this kung mag claim ako ng critical illness. Pero this is just your hospital allowance. hindi ba sapat na meron akong SOA from the hospital, and at the very least a clinical abstract.
Ang theory ko is they are just using a similar form para doon sa hospital benefit. Another is their way to tapping on my medical records early on para come I claim my riders eh puwede nila busisain.
Baka may underwriting conditions HMO mo regardless kung para saan ang claim?
hindi ba HMO is a form of health insurance plan? anyway, sana makuha mo since entitled ka nga naman. a way to protect themselves from fraud is to have the GP vouch for you and your condition.

Hospital errors can be made too, saka facility lang sila. Alam mo naman pag ganito they will try to poke all possible corners to check the validity.
