Provider asking for balance payment, and stuck in a loop when covered by two insurances

Hi - I have a little bit of a confusing situation and could use some advice on how to navigate.

In January I saw my psychiatrist as normal. However, I had switched jobs and he had billed the appointment to Aetna, my insurance at my old company. Aetna bounced it back to him but I guess he didn't see it in time, and resubmitted to Aetna again, instead of Blue Cross, my new company and would be considered my primary insurance. Finally in May he billed Blue Cross, which denied since it was out of 90 days of the claim window. However, the claim is being showed as "approved" on my Blue Cross dashboard, but behind the scenes they did not pay him. Aetna, the secondary insurance at my old company (through to the end of Jan, even after I quit) won't pay unless they see a denied claim from Anthem since its primary.

My doctor is asking for balance payment, but it feels weird since I was double covered for this appointment.

Any advice on how to proceed? I don't want to just not pay the balance with my psychiatrist as he has been seeing me for some time now and not paying would probably just cause he to not see me anymore.
