Should I purchase an extended car warranty for my used car?

Recently purchased a 2013 Nissan Altima which has a CVT which is prone to going out without warning.
The car is at 117k miles and these transmissions usually go out way before that so I may be safe if I maintain the car well. The repair usually costs 4K.
I got quoted $67/month for 36 months/250k miles. Can anyone recommend an extended car warranty that would cover the transmission which is my primary concern?
I really just need the peace of mind knowing I won’t have to pay 4k out of pocket.
