I don't think I should have a default on my credit file

I opened a phone contract in 2015 and in 2016 I unfortunately lost my then current job so I wasn't able to pay my bill for about 3-4 months. After the mispayments my account was closed and had to pay the remaining amount that was still left on my contract.

A debt recovery agency then contacted me and we arranged I make weekly affordable payments through direct debit. The payments then stopped being taken from my account in the summer of 2017, and with no contact from the phone provider or the debt recovery agency I assumed the account was fully paid off. But then when I checked my credit file about March 2018 there was a default put onto my file from the phone provider which started on the 1st January 2018.

If anyone can help me in what to do next whether to dispute it or not I would be extremely grateful as the default lasts for 6 years on my file and its really affecting my credit.
