Thygeson's Superficial Punctate Keratitis (TSPK) and blurriness with no flare up
Post Body

Hi all,

I wanted to ask my question here in case anyone knows more. I was diagnosed with TSPK when I was about 20, and I am 28 now. Getting the diagnosis took a while. The eye doctor's at the Bascom Palmer Eye Institute in Miami thought it was maybe a version of the herpes simplex, but after many tests and being shuffled around, they decided it was TPSK. Literally 3 or 4 doctors agreed in the same room while I was there, so I was confident in the diagnosis

During flare ups, I get all the symptoms: blurry eyesight, watery eyes, foreign object sensation, and, when it's really bad, photosensitivity. Sleeping both activates and helps it. It often gets triggered when first open my eyes in the early morning, but I usually just go back to sleep. By the time I wake up again, the watery eyes and foreign object sensation are usually gone and I'm just left with blurriness that resolves over the next few days. My right eye is far worse than left eye; I hardly get symptoms in the latter.

I've noticed over the years, though, that I might not have had flare up at night (or at least one that I recall) and I wake up with my right eye slightly blurry. The blurriness is never permanent, and usually resolves over the next few days (maybe even the next day). When I started to notice last year that the eyesight in my right eye would be blurry without any memorable TPSK flare ups, I went to see an ophthalmologist where I live now, since I started to get concerned. This doctor said 1) he didn't see evidence for TSPK and 2) that I had slight astigmatism, more so in might right eye. I believe the astigmatism diagnosis, since the eyesight in my left is like 98% acute, and the best days for my right eye are like 95% acute. I don't know why he said he didn't think I have TSPK, though, but I'm guessing it's because I wasn't flared up in that moment. When the doctors in Miami diagnosed me, I was having a major flare up. Also, I am not sure why, if I have astigmatism, some days my eyesight in my right eye is clearer than other days. He prescribed glasses, but I never got them because when I've had no flare up or my right eye isn't blurry for this inexplicable reason, I don't need help seeing clearly at all.

TLDR version: Astigmatism might explain why my eyesight is never 100% clear, but it doesn't explain why the degree of blurriness fluctuates in one eye. Can TPSK cause blurriness without any major flare ups? What else can cause temporary blurriness? Can astigmatism in one eye vary across time in the amount of blurriness it causes?
