Rejected release form

I sent in the release form and had it approved a year and a half ago but just received this notification today. Has anyone else dealt with this? I’m panicking that fans will lose content that they paid for. Does anyone know what format of release form is accepted? They didn’t give me much to go on and of course I didn’t receive an email either 🙄
