Improvement plan due to underperforming

So I've been in this company for more than a year, I enjoy my time working here until few months ago. I was told by my manager that I was underperforming and some other stuff. I was mention that I was abusing my medical leave and it was seen as a pattern cause I do it on Friday but only once a month. We don't need to provide medical cert. I only took the medical leave cause my mental health was taking over me. Ever since that day I had that discussion on the improvement plan with my manager, it got worse. I'm now seeking therapy. I now have to force myself to get a diagnose to save my ass as I need "proof".

Just a note, all of this improvement plan took place without checking on me on what's happening and just said, okay,this employee got issue. I'm now mentally and physically exhausted with working this employer. I don't know what to do anymore. It was once a place I enjoy but now,it's hell
