How to drill down a well and run a liner 4.5" in 5.5"

I want to drill a well down about 2,000' that has 5.5" casing and use 4.5" casing with turned down collars. I figure I can use a single derrick drilling rig but how does the drilling and completions work. How do you get a bit that has a diameter big enough to drill that far down and create a large enough diameter hole to cement and complete the well with 4.5" casing?
