I’m afraid I’m dying but I can’t afford medical help.

I’m not so much afraid of death, I’ve come to terms with that. I’m more afraid of the impact my death with have on those around me. I don’t want to hurt my wife, my parents, my brother and niece. I don’t know what to do.
