Is it not standard to chart by exception?

Travel nurse halfway through my first assignment. The floor I’m on charts waaaaay too much. And, they just copy whatever the last person charted over and over again. I’ll get a patient back a whole week or two later and they’re still charting the exact same stuff that I charted the last time I had the patient. And then I change my charting, and suddenly everyone is charting that. I notice they chart much less when they’re copying my charting (because I chart by exception), but they’ll still add in anything that’s ever been charted on. It’s the norm here, for example, to chart that the patient’s skin is WDL and then go on to chart “blanching” for their coccyx, heels, and elbows, etc. Even though their skin is perfectly fine. No redness/pinkness/etc. They also frequently chart things that they for sure did not assess. I don’t chart how the patient’s gait is unless I saw them walk. But, this floor just charts whatever they hear in report. Is this the norm for a lot of places? It’s very opposite of how I learned to chart/how I’ve always charted.
