Is person, place, time, and situation an order of precedence in the A&O system (context for the question is in the body of the post)?

I'm in nursing school and I recently learned about the alert and oriented system of mental status and I think I need some clarification.
Everywhere I look, A&Ox3 is always defined as the person being alert and oriented to person, place, and time. However, I can imagine several scenarios/reasons why someone might be able to answer a different combination of three of the four questions (i.e. person, time, and situation).
