How to ask a doctor for a painkiller

I realize I have been getting high on nitrous to deal with some emotional problems as well as physical pain, I dont know how to ask a doctor for a painkiller without feeling like a drug addict. I seldom ever take opiates. I only when I actually need them and now when I need them I feel like too much of a coward to just tell my ent that my face is fucking kill me. I tried 4 advil, tried naproxen, nothing seems to help. My cousin gave me an oxy that he has from a shoulder surgery but even that didnt help, my face is seriously fucking killing me
