IYO, What title had the biggest misdirect in regards to box art versus actual in-game design?

I was fondly recalling pick-in up a copy of Kid Nikki: Radical Ninja from a garage sale and thinking it was going to be glorious after seeing the cover art. I was utterly confused as to what I had bought after popping it in. What a fever dream that game ended up being. Any other misdirection you can recall, positive or negative?

the box even calls it "the Soviet mind game." in the 80s, that sounded like something very sinister.
