Top Surgery denied (vent and question)

Hello! My husband was in a major car accident just days before his top surgery was to happen several years back, and needless to say he missed his surgery as he spent over 6 months in and out of the hospital dealing with issues from the accident. He basically gave up on the idea of ever being able to have his surgery (cost, and lingering health issues).

I was diagnosed with breast cancer earlier this year, and because of all of what I am going through we reached out insurance deductible (incredibly fast, cancer is expensive!). It was suggested that we try again to see if he would be able to have his surgery... We found a surgeon who was happy to work with his existing issues, and we were both so excited for him to be able to finally pursue top surgery again!! (I am going to have to have a double mastectomy myself, and the one good thing coming from that was being able to give him the surgery he's been wanting and needing for close to 10 years now) but, we just found out today that the insurance denied his surgery....

Does anyone have any tips for dealing with insurance companies? How can we dispute this and hopefully get coverage?

On a more personal note, I feel absolutely horrible. And I am angry so angry. Why is it that I am losing my breasts against my will, but my husband who works so incredibly hard to support and care for me and our family... He is denied this surgery that will finally make him feel comfortable in himself. It's absolute bullshit.
