Skill works based on amount of HP your team has, when the skill is activated, split by thirds. The three activation points are more than 66% HP remaining, between 33% and 66% HP remaining, and less than 33% HP remaining.
Level 1: Increases ATK by 100/150/200%
Level 2: Increases ATK by 150/225/300%
Level 3 figures still need to be tested, but we can assume it's 200/300/400%
