Looking to get more from this late 1977 400.

Got this old dodge motorhome with 43k original miles on its 400 engine. 12mpg and pedal to the metal for 70mph. easy access to Engine. What should I change? I know I want more than a 3 speed auto. Best way to fix massive exhaust leak at manifold? (I see sparks hehe)
