Does the flex disc need to replaced?

It's an S204 C180CDI, 120HP, MANUAL.

Also, i have this two "problem".

First is, when i let the clutch out while shifting from 1st to 2nd, and sometimes from 2nd to 3rd, the car jerks a bit. When i release it slowly, it's fine, but in faster intersections, i have to be quick, and it does this jerking.

Also, the car vibrates a bit (feel on the steering wheel) above 100km/h (62 mph)

Do these problems have a relation with the flex disc, or the problem is in me, and somewhere else in the car?
