Part D and Insulin Coverage

I have been led to beleive that new rules limit Insulin Cost to $35/month.
In checking out potential plans, I am seeing $35 in the column for Regular Coverage, and Donut Hole Coverage, but ramps up to Several hundred dollars once you reach catastrophic coverage! Does the $35 limit not apply there?
