I have ED and I don't know what to do

I have ED and I'm a little embarrassed to go see a doctor about it. I don't have a family doctor so it would have to go to a clinic and I wouldn't know what to say to the nurse
I heard testosterone pills can work but I don't know if it's a scam or something that can help.
I'm a huge smoker and slightly overweight 26 year old male. ED is destroyed relationships in the past and I would like advice on what to do
