Penis foreskin tight/keeps splitting when I masturbate

I’m trans-femme and have been on estradiol for just under two years now.
This has been an issue for about half a year where I will masturbate and then it becomes painful to pull the foreskin too far back, and ultimately my foreskin splits and is painful for a while.
It’s totally fine pulling it back normally, but it happens during masturbation and then it’s painful to pull back for a while.
What should I do here?
