Somehow tore a sagittal band, am I looking at surgery?

46yr male, with several medical conditions and medications, due to length I have posted them at:

Few questions:

A. On May 28th, I flicked an ant off of my chest and somehow managed to tear a sagittal band in the middle finger of my left hand (yes, I have heard all the "ant man" jokes).

Per my doctor this usually occurs due to sports like boxing or rheumatoid arthritis however due to my medical conditions I am pretty sedentary and rheumatoid arthritis was ruled out over a year ago when I was tested while attempting to find a cause for my other pain issues.

Are we possibly missing any other potential causes?

B. As the sagittal band pain/swelling has lasted approximately 2 months while finger has been in a brace I had a MRI does last week, and findings are posted here: https://imgur.com/tkz0tn8

If I am reading this right, am I looking at surgery?

C: I have several other tendons on both hands that are "loose" and sliding off of the knuckles (but no tears yet) and my doctor has advise that it is possible I may have the same issue occur in the future. She does not know why this is occurring but has said "it could be genetic or could be hands have worn down".

Being only 46, is there any possible conditions that may have caused this/we should be looking at?

Is there any potential preventative measures we can do to prevent future sagittal band tears?
