Photos during massage?

Just booked a massage in which I was looking forward to however they've sent me a webform questionnaire about my health etc. At the bottom is a term saying I authorise photos being taken for promotional content.

I'm not able to complete their form without signing this and there's no option to disagree, just a tick box and signature. Is this normal? I've already paid so feel a bit rubbish knowing this as I'm not sure they will refund me if I cancel?

I don't feel I'd be able to relax knowing someone could be photographing me at any point. I'm very anxious anyway so often find it hard to relax even when having a massage.
