So about Visions Civil War equation

In the 8 years since Mr. Stark announced himself as Iron Man, the number of known enhanced persons has grown exponentially. And during the same period, the number of potentially world-ending events has risen at a commensurate rate”

"Are you saying it's our fault?"

"I'm saying there may be a causality. Our very strength invites challenge. Challenge incites conflict. And conflict... breeds catastrophe. Oversight... Oversight is not an idea that can be dismissed out of hand”

Let's break down these enhanced individuals and world ending events and see if Visions equation stands up. In the 8 years between Iron Man announcing himself to the world and the proposal of the sokovia accords there have been many threats to the world. Let's start with the two that would back up Visions equation, Justin Hammer and Whiplash, Hammer held a grudge against Tony and his success in life and sought to bring him down. The one problem with this working for Visions theory is that it also works against it, Hammer was doing it for a business reason and would have kept the funds. Hammer wasn't an evil terrorist or funding terrorists, just a corrupt business man who wanted to get even with Tony. His “ally” whiplash on the other hand does aid in Visions theory, because of his grudge against Tony and his need to kill him he upgrades his plans and creates a lot of collateral damage in his vendetta, Tony's power invited the challenge that whiplash needed to upgrade himself to take him down, this challenge created conflict in the form of the attack at monaco and the stark expo where people were attacked by killer drones which is a clear example of conflict breeding catastrophe.

Now these next few are where the equation fails. The US Military had an operation going where they tried to remake the same formula that created Captain America, this in turn created Hulk who had to go on the run because the military would force him to be a weapon. The search for him led to them giving another formula to Emil Blonsky who would betray them and take a serum of banners blood to become Abomination. I use this to discount Visions equation because it shows a governing body is not a reliable form of Oversight because if they had left Banner well enough alone the destruction caused by Abomination would never have happened. Whilst all this is going on over in New Mexico we have an alien incursion where an Asgardian weapon is attacking 5 Asgardians because Loki wanted to kill Thor because of his lust for power and desire for revenge against Odin, so we have the 2nd incident involving aliens arriving on Earth that could be counted as a “World ending event”, it even disproves Visions equation because Loki created that scenario not because Thor was this grand hero who needed to be challenged but because Loki did it largely unprovoked. Loki would again resurface leading an army of Chitauri to attack the Earth at the behest of Thanos, not because the Avengers existed but because he wanted to acquire the two stones on Earth and because Loki wanted revenge for failing the first time. This is the 2nd big world ending event and its not created out as a challenge to the avengers threats, it's just an alien invasion that the Avengers were instead created to counter, this is the major example of the Avengers themselves being the challenge created by the opponent's strength and it was better than the council's plan to nuke the whole city (yea what was that gonna do shower the city in hugs). Captain America would go on to prevent a major threat to the world when he uncovers Hydra is still alive and imbedded in shield and the government (again a reason why you shouldn't trust an organisation with agendas that go beyond the avengers mission to help people), this being a threat Captain America was created in response to again disproving Visions equation (also i would love to point out Tony did help the bad guys here because he upgraded the helicarriers that Hydra were going to use to subjugate humanity, bet we won't see another time where Tony ends up making the situation worse by the time I finish this). In England we see another alien invasion and this time as Vision predicted there is a world ending event in progress, the dark elves have uncovered the reality stone and seek to plunge the universe into darkness, not because they wanna kill Thor because he wasn't even born when they had this plan. We finish with Iron man who faces off against AIM an organization created by Aldrich Killian who like Hammer wanted to make a tidy profit, he would do this by creating an army of supersoldiers funded by a the government at the help of a corrupt politician (hey look another reason why the safest hands are still your own) who he would place as president whilst he runs the war on terror as both terrorist and man supplying the soldiers to fight the terrorists making a profit of both. He had no intention to involve Tony Stark until Tony involved himself and that led to his demise and proves Vision wrong, this is a villain who Tony actively on camera challenged because of his villainy. Last villain to mention and also world ending event is Ultron, Ultron was created by Tony (Oh look he does pop up again) who after having a pretty bad hallucination decides to go behind everyones back and create an Ai to live in robot bodies to protect humanity, when this plan ultimately goes wrong because rather than do proper thorough work and be there for its finish Tony was having a party and Ultron goes on to ultimately threaten the world and launch a devastating attack on Sokovia, we can clearly see that Ultron was the fault of Tony and he wasn't really trying to kill them solely as his main goal, he just wanted to wipe out the world.

So through all this we can very clearly see that Vision is not correct as only the events of Iron Man 2 back up Visions equation whilst every other movie disproves it. The Avengers do not need oversight. They need training yes because if Wanda had thrown Crossbones at an angle maybe he wouldn't have killed anyone but himself when he exploded, but they do not need governments overseeing them with their agendas and furthermore lack of knowledge on how an enhanced person can operate. Blaming the destruction caused by the sokovia incident on the avengers is just wrong. They couldn't have done any better because they had limited numbers and had to prioritise, civilians being killed during evacuation is devastating but honestly it's the lesser of the evils present. In short Vision is entirely wrong about his idea and support of the accords, Tony is the one who needs proper oversight and all of this is without even properly mentioning the threat to Earth from the Kree threat in the 90’s and Egos expansion which again disprove Visions theory.
