Looking for info on getting a card

Hi all! I have a few questions about getting a MMC. I was recently diagnosed with facet degenerative changes in my lower back and mild cervical spondylosis in my neck after months of pain so i’m hoping it would help.

Anyway, i’m wondering how this all works. I searched the sub and saw people went through greenleaf and other doctors. Do i ask my doctor? Or do they notify my doctor? My spouse is also military does that change anything? Obviously, he will not be using it but i don’t want it to affect him (i’m assuming it won’t) just want to make sure.

Do i share my medical records?

Any advice is appreciated

I filled out a thing with teleleaf saying I had anxiety, paid, and submitted. They called me the following day at the appointed time and said it was all good and submitted the prescription to the pharmacy. It was incredibly easy. I'm 100% positive you'll be approved. Their "doctor" calls you. I can definitely confirm that they don't deep dive at all.
