Lisboa: A Municipality in a Metro Region?

Oi!
I’m having a bit of trouble understanding how Lisbon is governed.
It seems that the most influential form of government is at the national level. Additionally, Lisboa is then governed as a metro area with 18 other municipalities (área metropolitana de Lisboa) and governs itself as a city.
Is the “metro government” influential? Is this system efficient? Can Lisboa make independent decisions?
