Relatives need help placing severely disabled adult child

This is in Philadelphia, Pennsylvania. My aunt (67) and grandmother (91) are the main caretakers of my aunt's daughter (~29). I will call the daughter S for the purposes of this question. S has Downs Syndrome and functions at the mental capacity of a 3 or 4 year old. She also has some other unknown issues that cause her to act out and become extremely violent. She has reached the size of 345 pounds at the height of 5 foot 1. No one in our family is capable of handling her in her rages, and we can't get any help from the city or state to get S placed in some kind of care facility. The police have been called multiple times and won't do anything. S has been abusing my grandmother especially. I told my parents to try a disability attorney but I also wanted to ask here because I know alot of you are social workers or have worked with families with issues like these. What can we do?
