Nursing facility failed to reach out during the decline, death and cremation of a loved one.

Hello, I'm in the state of Michigan. I have a grandmother who suffered from dementia and was placed in a nursing facility due to needing 24/7 care. Myself and the family that were taking care of her had a few personal situations of our own, and have not been to visit her since early August. We called around Halloween to check on her and were told she expired in August. My sisters and uncle are the emergency contacts, no one was contacted. They do not have a record of attempting to contact anyone either, they have admitted this, even though they are supposed to do so legally if there are any status changes in health. To add insult to injury they had her cremated and neither the nursing or funeral homes contacted the family or followed the plan put in place. We are extremely disappointed by the situation. Im not sure what our next step is.
