I am representing myself in this matter, as otherwise I wouldn’t be able to do this.

I just want to ensure I have all the proper documentation. I am pretty confident I do, but I can’t help but doubt myself as I’ve never done anything like this before.

I have prepared :

-petition for minor name change (soon to be notarized)

-Certified copy’s of birth certificates

-State photo ID with current address

-Proof of Residency

-Federal & State background checks

-Court papers stating I have full custody and rights

-Court papers showing where listed father has been incarcerated and took a guilty plea due to domestic violence and child abuse/ battery (he was released this year if that matters)

-Court papers stating listed father is not allowed contact with myself nor the minor children in question indefinitely
