Hello all. My provider for a chronic condition recently quit for which I received no notification. His practice group did not assign his patients to a new provider automatically. As a result, I need a new first-time checkup for my condition with a new provider at the practice. before I can renew my medication. I was able to get a new provider there but was unable to get an appointment for 4 months. Is this a violation of continuity of care laws?
