If I become mentally disabled…

Is there any kind of legal form, almost like a DNR that would allow for me to die in case of a terrible accident/stroke that left me mentally disabled or unable to care for myself? Like basically if I can’t feed myself, I don’t want anyone to do it for me, just let me starve. Or if I have a stroke and it’s like 90% certain I will be brain damaged, to not put me on any kind of life support or do any life saving measures, just let me go?

This is my worst fear. Nothing scares me more than not being able to care for myself, I would 100% rather just die.
