Wife in chronic pain

Wife in chronic pain and on daily opiates. On most days that is fine but some days she still has major break through pain. She never takes more than of what is prescribed because

a. She would run out at the end of the month and can not get refills and

b. She is a recovering addict and any dosing over her prescribed amount would be a break is sobriety.

I want her to try this kratom on days when she had the breakthrough pain. Does anyone have anything they could share that might help? Dosing, strains. Do’s and don’ts. Interaction with other drugs? Etc.
