PI absolutely scorches when playing 4k, or 2160p

It doesn't seem to do this nearly as much when it's streaming the file from my media server, but if I connect media and play anything UHD I can barely touch the heatsinks.
It's a pi 4, 4gb, with 3 heatsinks, and a small fan. The fan isn't really working consistently anymore, but even when it does it's not enough.
Are those heavier duty cases with cooling worth the investment?
