How do y'all avoid the loose knit column along the left side of ribbing or cables?

So, I've noticed that anything I'm making something that has wide ribbing (at least 3x3 or wider) the left most knit column just before I start the purl stitches always gets really loose. How do we avoid that happening? I've tried pulling the stitch extra snug before moving on the purl, but they're still loose. I know I must be missing something. Also, does anyone know why this happens?
