looking for something i saw on here a few days ago.

it was something you can print off and give to people you would be willing to make things for so they can give you their measurements. It was super cool, but i didn't remember to save it, and now that I have my new computer and printer, I can't seem to find it. I tried scrolling back about a week in the new tab, but i seem to just keep missing it. Anyone one of you lovelies happen to have saved it?
