How the fuck does saving work?

Mild spoilers ahead but earlier I played for 3 hours and my game never saved once, I was under the impression it saved when you slept or finished/advanced a quest? Welll I finished off multiple bandit camps, took Theresa out on a date where I had to whoop some guys ass, and slept multiple times at my bed in the mill and it never saved once! Finally got it to save by renting a room at an i nn, theres gotta be a better sure fire way to have it save(minus the thing you can drink to save the game)
