How do I replace/extend this?

I’m trying to replace this box with an elbow. The prior owners put a drop ceiling below it and the basement is noticably watmer since opening this area up to bring heat in. However, as you can see in the photo, there is a joist of some sort that was put in as well so I can’t just unscrew it and pull it off. Also, the venting that was on there is flat, and the vent box if I could just extend it would be great too, but it needs to come down 3-4” so it would come through the tile. I Would appreciate any help. Thanks!
