How to move from 2400 MMR to 3000

Hi there.
I'm feeling like I've plateaued a bit lately, hovering around ranks 9-12 and I'm really trying to figure out what makes the difference between a 2500 MMR player and a 3000 MMR player. You know, past that point when you understand the game and can competently position and the like. Without watching replays obviously, what separates the decent players with the actual good ones?
