Why are these staying on every run?

Both Immortals I got in previous runs, they always say then now. I definitely didn't get them this run. Why?
Also completely unrelated question. In the Battlements, are there some things just out of reach depending on RNG even if you have glass walks? There are some areas that some unreachable some runs. Like the ground will crumble, no glass walks appear, and I see not glass walks leading from anywhere else.
