Help! Hemorrhoids or??

Been dealing with these for years. Within the last month it’s gotten worse and last week they were pretty painful. But they itch so so bad. Mostly at night but all day they constantly itch. I scratch them a lot and wipe harder due to the itching relief. What can I do to help these get better or should I just go see a doctor?
