Game bug that needs to be fixed

If your not careful in the campaign theres just a random chance that when you ground pound with the grapple (usually on a hill) youll fly through the ground and then fall to your death, the amount of times i had this happen while on my playthrough was enough for it to be far from okay. Please, someone fix this
