Is there a way to control followers in combat somehow?

Their AI seems really bad in combat. I rarely see them block or dodge. Things seem to just chain stagger or knock them down or they die quickly. Is there a way to tell them to keep their distance and used ranged attacks or to be defensive? I spend most of my time trying to babysit them or just having them die and then trying to kill 5 enemies by myself.
EDIT: This is on Hard to Extreme difficulties also.
